AIDS and HIV Research Project

I, _______________________ , this _____ day of ______________ , 2002, hereby acknowledge that I have entered into collaboration with The Colloidal Silver Database Website HIV/AIDS Research Project ( Project ) as an individual and independent research partner conducting experimental and personal research.

I hereby accept personal and full responsibility for all actions and results associated with the Project. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless all participants of the Project, their officers, officials, and agents, with respect to all and any injury, disability, or loss or damage to person or property, whether arising from the negligence of the participants or otherwise.

Further, I agree that neither I nor any agent of the Project is practicing medicine, including but not limited to the diagnosis or treatment of any medical condition.

I agree to conduct my personal research to the Project's standards and according to the mutually agreed upon protocol(s) as long as I am a participant of the project. I agree to protect the anonymity of all parties involved. I agree that all information reported as a part of the Project will be truthful and accurate, and I release this data to the Project for unlimited use. I understand that my personal information will be held confidential, unless otherwise specified in writing.

I understand that participation in the Project is mutually voluntary, and that any party may withdraw without notice and for any reason.
